Understanding disc-related pain
The discs are strong fibrocartilaginous structures that sit between the bones of the spine. They act as shock absorbers, help distribute load, and allow the spine to move while remaining stable.
Disc-related pain occurs when a spinal disc becomes irritated or injured. This may include a disc bulge, protrusion, extrusion, or herniation, and in some cases the disc may irritate a nearby spinal nerve. Depending on the location, this can cause symptoms in the neck, arms, lower back, or legs.
It's important to remember that not every disc change causes pain. Many people have disc bulges or age-related changes visible on MRI without any symptoms. For this reason, imaging findings should always be interpreted alongside a detailed clinical assessment.
The aim is not simply to identify a disc injury, but to determine whether it is responsible for your symptoms and what factors may be contributing to the problem.
Common disc-related conditions
Disc injuries can present in different ways depending on the location and severity of the irritation.
Common presentations include:
Lumbar disc injuries, causing low back pain with or without pain into the buttock or leg.
Sciatica, where irritation of the sciatic nerve produces pain, tingling, numbness, or altered sensation travelling into the leg.
Cervical disc injuries, which may contribute to neck pain, shoulder pain, or symptoms travelling into the arm or hand.
Degenerative disc disease, where gradual age-related changes may reduce disc height and alter spinal mechanics.
Acute disc injuries, often developing after lifting, bending, twisting, or sudden loading.
Every presentation is different, and the severity of symptoms does not always match what is seen on a scan.
Why do disc injuries develop?
Disc injuries rarely occur because of a single movement or event. They are often the result of repeated loading, reduced tissue capacity, and changes in how forces are distributed through the spine over time.
Factors that may contribute include:
Repetitive bending, lifting, or twisting
Prolonged sitting or sustained postures
Reduced spinal and hip mobility
Previous episodes of back or neck pain
Sudden increases in physical workload or training
Reduced physical conditioning or movement variability
Smoking, which is associated with reduced disc nutrition
Age-related changes in disc structure
Understanding why the disc has become overloaded is an important part of reducing future flare-ups.
What does assessment involve?
A detailed assessment aims to determine whether your symptoms are arising from the disc itself, irritation of a spinal nerve, surrounding joints and muscles, or a combination of these factors.
Assessment may include:
Spinal movement and functional testing
Neurological examination, including strength, sensation, and reflexes
Assessment of nerve mobility and irritation
Hip and pelvic mechanics where relevant
Review of previous imaging findings
Screening for signs that require further medical investigation
